	1. Tell us who you are! Indicate your name (Referrer), NRIC and contact no. on the "MY REFERRER DETAILS" section of the application form.
	2. Refer your buddy! Hand out forms to friends (Referee) for their completion.
	3. Mail us! Get them to sign and send the completed form back to us together with the following: Photocopy of Referee's NRIC (front and back) Referee's income document(s) (see "Mandatory Documents")
	4. Be rewarded! Receive your Reward(s) within 1 month after the end of the promotion.

Terms & Conditions 

• Promotion is valid till 31 March 2012
• All referral Credit Card applications submitted under this Promotion must be approved by Maybank by the promotion end date
• Only existing Maybank Credit Cardmembers (“Referrer”) are eligible to participate in the Promotion
• Referrer must refer contacts (“Referee") to Maybank via the stipulated Member Get Member Credit Card application form to be eligible to receive the reward(s) (“Reward(s)”)
• Each eligible Referrer will receive Reward(s) based on the following incentive tiers:
  • 1st Tier - Referrers who refer 2-3 Referees will receive S$10 worth of Takashimaya voucher for each approved application
  • 2nd Tier - Referrers who refer 4-6 Referees will receive S$20 worth of Takashimaya vouchers for each approved application
  • 3rd Tier - Referrers who refer 7 Referees will receive S$160 Takashimaya vouchers
  • For subsequent approved applications - Referees in this tier will receive S$20 worth of Takashimaya vouchers for each approved application
• Eligible Referrers will receive a redemption letter and/or vouchers stating the Rewards earned within one month after the Promotion end date
• The Rewards are limited to the first 1,000 successful referrals
• Referrer will not be entitled to the Reward if the Referee is one of the following:
  • An existing Maybank Credit Cardmember
  • Has cancelled his/her Maybank Credit Card within 6 months prior to the start of this Promotion
  • Applying for a Maybank Supplementary Card
• In the event the Referee is referred to Maybank by more than one person, the first person who referred the Referee as determined by Maybank at its discretion will be eligible to receive the Reward
• The Referrer will only be deemed as having made one successful referral regardless of the number of Credit Card accounts applied for by the Referee
• Maybank reserves the right to vary or substitute the Activation Gift and all Rewards offered under this Promotion at its sole discretion without prior notice to any persons
• Maybank reserves the right to vary, delete or add to any of these terms and conditions, or withdraw, suspend or discontinue the Promotion at any time without any notice or liability to any person
• Maybank shall not be liable for any loss or damage whatsoever or howsoever caused arising from the use of the Activation Gift and any Rewards issued under this Promotion and shall not be involved in any dispute relating thereto
